Assistance with life stage transitions includes both short-term and long-term support aimed at enhancing and empowering the participant’s abilities and choices. Major life transitions, such as moving out of home, starting university, or securing a job, are part of everyone’s journey. These changes can be challenging, both emotionally and practically, and are often more difficult for individuals with disabilities. Adjusting to new environments and situations can be tough, but through this NDIS support category, Sensible Care offers a range of services to support life stage transitions. Our goal is to make the transition process as seamless as possible, while ensuring the participant’s comfort is maintained. We collaborate with the participant to facilitate a smooth move, encouraging them to adjust to their new surroundings and develop a sense of ease.
